The pyrrolizidine alkaloid (PA) constituents found in some butterbur products are substrates of CYP3A4. Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites.

Here's the twist with this pairing: it's not your primidone that gets affected, it's the butterbur. Primidone speeds up a liver enzyme called CYP3A4. The worry is that some butterbur products still contain natural compounds called pyrrolizidine alkaloids (PAs), and that same enzyme can turn PAs into byproducts that are hard on the liver. So by revving up CYP3A4, primidone could, in theory, increase how much toxic material your liver makes from those PAs.

I'll be honest, this is a theoretical concern based on how these substances behave, not proven harm in people. The simple safeguard is to use only PA-free butterbur products. Before combining the two, check with your pharmacist or doctor so they can confirm it's a clean, standardized product.

What Is Primidone Used For?

Brand name: Mysoline

Primidone tablets — sold as Mysoline or under the generic name primidone — are used to control seizures in people with epilepsy. Both Mysoline and primidone tablets are approved for grand mal (generalized tonic-clonic), psychomotor, and focal (partial) epileptic seizures.

Primidone can be used alone or alongside other anti-seizure medicines. It may also help control grand mal seizures that haven't responded well to other anticonvulsant treatments.
