Mephenytoin falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Butterbur interacts with.

Theoretically, CYP3A4 inducers might increase the toxicity of butterbur.
The pyrrolizidine alkaloid (PA) constituents found in some butterbur products are substrates of CYP3A4. Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites.

Here's the angle on this one. Mephenytoin (Mesantoin) is a seizure medicine that can rev up a liver enzyme called CYP3A4. By itself that's normal, but it matters when you pair it with butterbur. Some butterbur products still contain pyrrolizidine alkaloids (PAs), and your liver uses that same CYP3A4 enzyme to turn PAs into harmful, liver-damaging byproducts. So in theory, mephenytoin speeding up that enzyme could mean more of those toxic compounds forming.

I'll be honest, this is a theoretical concern based on how the chemistry works, not solid human data. The simplest safeguard is to choose a PA-free butterbur product, which sidesteps most of the worry. Either way, please run it by your pharmacist or doctor before starting.
