Lactobacillus acidophilus is generally well tolerated in healthy adults and children. Gastrointestinal side effects like stomach discomfort, belly pain, indigestion, gas, bloating, diarrhea, vomiting, and burping can occur when taking it with other probiotics, but these are uncommon.

In rare cases, live probiotic organisms have been linked to serious infections like blood infections and abscesses, mostly in people whose immune systems are already compromised or who have underlying conditions — this risk is low in otherwise healthy people. One trial reported rash and itching with a probiotic combination, though it's unclear whether L. acidophilus was the cause.
