Berberine is generally well tolerated in the short term but commonly causes digestive upset—abdominal pain, bloating, constipation, diarrhea, gas, nausea, and vomiting. It can also cause dizziness, drowsiness, fatigue, and headache.

Bergamot extract taken by mouth appears well tolerated short-term, though one case of heartburn was reported. The safety data advises against berberine in pregnancy and while breastfeeding, as it may cross the placenta and pass into milk and has been linked to harm in newborns.

Bergamot also lacks enough safety data in pregnancy and breastfeeding, so avoidance is the safer choice. Note that bergamot's essential oil is toxic if swallowed and can cause sun-related skin reactions; the extract in this product is different, but sun exposure should still be limited.

Berberine can reduce metabolism and increase serum levels of cyclosporine. Berberine might inhibit cytochrome P450 3A4 (CYP3A4), which metabolizes cyclosporine.

Following a reduction of the tacrolimus dose to 3 mg daily, blood levels of tacrolimus decreased to 12 ng/mL.
