Does it work?
Moderate evidence
Lutein and zeaxanthin are possibly effective for age-related macular degeneration (AMD) and lutein for cataracts, based on the evidence we hold. Both show insufficient evidence for other conditions listed, including cognitive decline and Alzheimer's disease.
Astaxanthin's effectiveness for any condition — including AMD, cognitive decline, and athletic performance — is not established in the data we have; the evidence is insufficient to rate it. Anthocyanins' effectiveness cannot be assessed from our data.
How safe is it?
Well-documented data
Lutein is generally well tolerated at typical supplement doses up to 20 mg daily with no adverse effects reported. It's likely safe in pregnancy and lactation, though supplement safety during breastfeeding hasn't been well studied, so check with your doctor.
Astaxanthin is generally well tolerated in studies, but long-term safety data are limited. The most common side effects are abdominal pain, diarrhea, and red-colored stools; higher doses (40 mg daily) have caused severe stomach pain in a small number of trial participants.
Astaxanthin should be avoided in pregnancy and while breastfeeding — there isn't enough safety data. Zeaxanthin is generally well tolerated at typical doses and is likely safe in pregnancy and lactation, though supplement safety during breastfeeding hasn't been well studied.
No adverse effects are documented for zeaxanthin.

Zeaxanthin carries a Moderate interaction with diabetes medications (antidiabetes drugs). In animal models, zeaxanthin lowered blood sugar, so taking it alongside diabetes drugs might increase the risk of low blood sugar (hypoglycemia).
Population studies haven't confirmed this risk from dietary sources, but the theoretical concern remains.

Theoretically, taking zeaxanthin with antidiabetes drugs might increase the risk of hypoglycemia.
In an animal diabetic model, zeaxanthin has hypoglycemic effects. However, population research has found that increasing intake of dietary zeaxanthin plus lutein does not decrease the risk of developing type 2 diabetes.
