Does it work?
Moderate evidence
The evidence for Bacillus coagulans is strongest for constipation and irritable bowel syndrome (IBS), where it's rated possibly effective — meaning there's some promising research, but it's not definitive yet. For antibiotic-associated diarrhea, athletic performance, cancer, and fatty liver disease, the evidence isn't established enough to rate.
Backillus subtilis shows possibly effective evidence for antibiotic-associated diarrhea. The rest of its uses — athletic performance, cirrhosis, constipation, eczema, and diabetes — have insufficient evidence in our data.
We don't hold effectiveness ratings for MicrobiomeX or Bacillus clausii, so we can't comment on what those components do.
How safe is it?
Well-documented data
Both Bacillus coagulans and Bacillus subtilis are generally well tolerated in healthy adults, though the facts note that product quality and safety can vary. The data on Bacillus subtilis includes one reported case of headache in a clinical trial and rare cases of diarrhea, plus historical cases of serious infection (bacteremia and meningitis) linked to fermented foods with a related strain in people who were severely ill or immunocompromised.
Probiotic products in general carry a theoretical risk of serious infection (bacteremia or sepsis) in people with central lines, severe illness, or weakened immune systems — including premature infants — though this is uncommon in otherwise healthy people with intact immune systems. There isn't enough safety data overall, so check with your doctor or pharmacist before starting, especially if you have a central line, are severely ill, or have a weakened immune system.

Bacillus coagulans SNZ-1969
Antibiotic Drugs
Theoretically, taking antibiotics with Bacillus coagulans might decrease the effectiveness of B. coagulans.
B. coagulans preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. coagulans preparations by at least two hours.
Bacillus subtilis 1972
Antibiotic Drugs
Theoretically, taking Bacillus subtilis with antibiotic drugs might decrease the effectiveness of B. subtilis.
Since B. subtilis preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. subtilis preparations by at least 2 hours.
