This tablet contains 2 active ingredients: vitamin B6 and L-carnitine (CarniPure™). Vitamin B6 is a B vitamin needed for nerve function, immune health, and processing amino acids.
L-carnitine is a compound your body uses to turn fat into energy, especially in muscle and heart cells. The product also contains several inactive ingredients—dicalcium phosphate, cellulose, modified cellulose, cellulose gum, vegetable stearin, and magnesium stearate—which are binders and fillers that help form and stabilize the tablet.
Does it work?
Strong evidence
Vitamin B6 is effective for treating sideroblastic anemia and vitamin B6 deficiency, and it's likely effective for lowering high homocysteine levels. It's possibly effective for pregnancy-related nausea and vomiting, though use in pregnancy should be under medical guidance.
L-carnitine is effective for treating L-carnitine deficiency and is possibly effective for high cholesterol, congestive heart failure, and angina. The evidence for L-carnitine's benefits in these conditions is not yet conclusive.
How safe is it?
Well-documented data
Vitamin B6 is well tolerated at doses under 100 mg daily and generally safe at normal dietary and supplement amounts. However, high doses over time can damage nerves (sensory neuropathy), especially above 1,000 mg per day.
Common side effects at any dose include headache, nausea, heartburn, and loss of appetite. L-carnitine is generally well tolerated at typical doses but can cause stomach upset, diarrhea, nausea, and a fishy body odor at higher amounts.
A rare serious effect is seizures. For pregnancy safety, vitamin B6 is likely safe, though high doses should be avoided without medical guidance.
Safety data for L-carnitine in pregnancy is not on file; lactation data for vitamin B6 is likely safe, while L-carnitine is possibly safe during breastfeeding.

Vitamin B6 also has moderate interactions with blood pressure medications (antihypertensive drugs), where it may add to their effect and lower blood pressure further, and with amiodarone, a heart rhythm drug, where it may worsen sun sensitivity. At lower doses, vitamin B6 may reduce the effectiveness of levodopa (a Parkinson's medication), though this doesn't happen if you're also taking carbidopa with it.
L-carnitine carries moderate interactions with blood thinners—warfarin and acenocoumarol—where it may boost their anticoagulant effect and increase bleeding risk. It also theoretically may decrease how well thyroid hormone replacement works by blocking the hormone from entering cells properly.

Vitamin B6
Amiodarone (Cordarone)
Theoretically, vitamin B6 might increase the photosensitivity caused by amiodarone.
Despite initial case reports suggesting that pyridoxine may have a protective effect against amiodarone-induced photosensitivity, preliminary clinical research suggests that pyridoxine may actually exacerbate this adverse effect.
Antihypertensive Drugs
Theoretically, vitamin B6 may have additive effects when used with antihypertensive drugs.
Research in hypertensive rats shows that vitamin B6 can decrease systolic blood pressure. Similarly, clinical research in patients with hypertension shows that taking high doses of vitamin B6 may reduce systolic and diastolic blood pressure, possibly by reducing plasma levels of epinephrine and norepinephrine.
Phenobarbital (Luminal)
High doses of vitamin B6 may reduce the levels and clinical effects of phenobarbital.
Preliminary clinical evidence suggests that vitamin B6 200 mg daily can reduce plasma levels of phenobarbital, possibly by increasing metabolism. It is not known whether lower doses have any effect. Advise people taking phenobarbital to avoid high doses of vitamin B6.
Phenytoin (Dilantin)
High doses of vitamin B6 may reduce the levels and clinical effects of phenytoin.
Preliminary clinical evidence suggests that vitamin B6 200 mg daily can reduce plasma levels of phenytoin, possibly by increasing metabolism. It is not known whether lower doses have any effect. Advise people taking phenytoin to avoid high doses of vitamin B6.
Levodopa
Vitamin B6 may increase the metabolism of levodopa when taken alone, but not when taken in conjunction with carbidopa.
Vitamin B6 (pyridoxine) enhances the metabolism of levodopa, reducing its clinical effects. However, this interaction does not occur when carbidopa is used concurrently with levodopa (Sinemet). Therefore, it is not likely to be a problem in most people.
CarniPure(TM)
Acenocoumarol (Sintrom)
Theoretically, L-carnitine might increase the anticoagulant effects of acenocoumarol.
L-carnitine might enhance the anticoagulant effects of acenocoumarol, an oral anticoagulant similar to warfarin, but shorter-acting. There are at least two case reports of INR elevation with concomitant use. In one case, a 33-year-old male with a previously stable INR had an elevated INR of 4.65 after L-carnitine was started and continued for 10 weeks. INR normalized after discontinuation of the L-carnitine-containing product.
Thyroid Hormone
Theoretically, L-carnitine might decrease the effectiveness of thyroid hormone replacement.
L-carnitine appears to act as a peripheral thyroid hormone antagonist by inhibiting entry of thyroid hormone into the nucleus of cells. Taking L-carnitine also seems to diminish some of the symptoms of hyperthyroidism.
Warfarin (Coumadin)
Theoretically, L-carnitine might increase the anticoagulant effects of warfarin.
L-carnitine might increase the anticoagulant effects of acenocoumarol, a shorter-acting oral anticoagulant similar to warfarin. There is not enough information to know whether this interaction occurs with L-carnitine and warfarin.
