This powder contains two active ingredients: L-citrulline and DL-malate blended in a 2:1 ratio. L-citrulline is an amino acid your body converts to another amino acid called L-arginine, which plays a role in blood vessel function and blood flow.
DL-malate (a form of malic acid) is a compound naturally found in fruits and involved in cellular energy production. There are no inactive ingredients listed.
Does it work?
Moderate evidence
L-citrulline is rated possibly effective for athletic performance — meaning there's some evidence it may help, though more research would strengthen that claim. For sarcopenia (age-related muscle loss), the evidence suggests it's probably not effective.
For heart failure, cardiovascular disease, Reye syndrome, and diabetes, we don't hold enough reliable evidence to rate it either way. DL-malate is rated possibly effective for dry mouth, but for acne, GERD, fatigue, fibromyalgia, and psoriasis, the evidence is insufficient to say whether it works.
How safe is it?
Well-documented data
L-citrulline is generally well tolerated in healthy adults, though long-term safety hasn't been fully studied. The most common side effects are gastrointestinal discomfort and heartburn.
A small number of trial participants with pulmonary hypertension reported cough. DL-malate is generally well tolerated in food amounts, but supplement safety isn't well studied — gastrointestinal effects like diarrhea and nausea have been reported with certain malic acid combinations, though it's unclear if malic acid alone was responsible.
The safety data advises against L-citrulline use during pregnancy and breastfeeding. For DL-malate, supplement doses haven't been studied, so caution is advised in those settings.

L-Citrulline
Antihypertensive Drugs
Theoretically, concomitant use of L-citrulline with antihypertensive drugs might have additive effects and increase the chance of hypotension.
L-citrulline is converted to L-arginine, which can increase nitric oxide and cause vasodilation. However, a meta-analysis of 5 small clinical studies suggests that taking L-citrulline 3-6 grams daily for 1-8 weeks does not lower blood pressure when compared with control.
Phosphodiesterase-5 Inhibitors
Theoretically, concurrent use of phosphodiesterase-5 (PDE-5) inhibitors and L-citrulline might result in additive vasodilation.
L-citrulline is converted to L-arginine, which can increase nitric oxide and cause vasodilation. Theoretically, taking L-arginine with PDE-5 inhibitors might have additive vasodilatory and hypotensive effects. However, in studies evaluating the combined use of L-arginine and sildenafil for erectile dysfunction, hypotension was not reported.

Theoretically, malic acid might increase the risk of hypotension when taken with antihypertensive drugs.
Animal research shows that malic acid isolated from tagetes roots can reduce mean arterial blood pressure.
