L-glutamine is effective for sickle cell disease. For HIV/AIDS-related wasting, postoperative (after-surgery) recovery, and critical illness from trauma, the evidence rates it as possibly effective — meaning there's some supporting research, but it's not conclusive.

How safe is it?
Well-documented data
L-glutamine is generally well tolerated in healthy adults. The most common side effects are gastrointestinal: belching, bloating, constipation, flatulence, nausea, vomiting, diarrhea, and stomach pain.
Headache and musculoskeletal pain have also been reported. If you have kidney or liver disease, use it only under medical supervision.
During pregnancy, L-glutamine is rated likely safe. For breastfeeding, there isn't enough reliable safety data, so avoid supplement doses unless your doctor advises otherwise.
People with bipolar disorder should be cautious: two cases of mania or hypomania have been reported in patients taking glutamine up to 4 grams daily.

Usage: Mix 1/2 teaspoon (2 g) into 6 to 8 oz. of fluid and drink when stomach is empty or as directed by your qualified healthcare professional. For athletes, maximal results will be obtained by supplementing with 2 g of glutamine after exercise.

L-Glutamine - the most abundant amino acid in the human body - is involved in many metabolic processes, including the synthesis and protection of muscle tissue, the production of glycogen, as well as immune support during periods of immune and muscular stress. L-Glutamine is also a major source of fuel for enterocytes (intestinal cells) and hence supports the integrity of the intestinal lining.
