Nattokinase is generally well tolerated in the short term, but because it thins your blood, it carries caution for anyone with a bleeding risk. Rare cases of bleeding, including brain bleeding, have been reported — especially when combined with blood thinners or aspirin.

There isn't enough safety data to recommend nattokinase during pregnancy, so it's best avoided then. Safety while breastfeeding is unknown, so avoid it during that time as well.

If you have a sensitivity to natto or soy, you may be at risk for allergic reactions including anaphylaxis; symptoms can appear within 2 hours of taking it.

Cautions/Interactions: Nattokinase may interact with anticoagulant and antiplatelet medications, including NSAIDs, warfarin, heparins, and clopidogrel.
