This product contains one active ingredient: nattokinase (NSK-SD), an enzyme derived from natto (fermented soybeans) at 36 mg per softgel. Nattokinase is a fibrinolytic enzyme — that is, it works to break down fibrin, a protein involved in blood clotting.
The softgel also contains inactive ingredients: soybean oil, gelatin, glycerin, soybean lecithin, glycerin fatty acid ester, and beeswax — these are excipients that hold and deliver the active ingredient.

Nattokinase is generally well tolerated in the short term, but because of its blood-thinning effects, it carries a caution for anyone with a bleeding disorder or high bleeding risk. Rare adverse effects include bleeding — including a serious case of brain bleeding when combined with aspirin — and hypersensitivity reactions (ranging from throat swelling to anaphylaxis) in people allergic to natto or its components.
Safety data in pregnancy is not enough to know either way, so the advice is to avoid it during pregnancy. Breastfeeding safety is also unknown, so it's best avoided then too.
