Nattokinase is generally well tolerated in the short term, but its blood-thinning effect means you need to be cautious if you have any bleeding risk. Rare cases of bleeding — including brain bleeding — have been reported.
Some people also have allergic reactions to natto components, with symptoms like throat or larynx swelling appearing within 2 hours of taking it. There isn't enough safety data to use nattokinase during pregnancy, so it's best avoided then.
Safety while breastfeeding is unknown, so avoid it to be safe.
