Nattokinase is generally well tolerated short-term, but its blood-thinning effect means caution is warranted if you have any bleeding risk. Rare reports of hemorrhage have been linked to oral nattokinase, including a documented case of brain bleeding in someone who combined it with low-dose aspirin.

There's also a small risk of allergic reactions in people sensitive to natto or its components; symptoms can include throat and larynx swelling and occur within a couple of hours of taking it. Nattokinase has thrombolytic and anticoagulant effects. There is a case report of intracranial hemorrhage in a patient who added nattokinase to aspirin therapy.
