Both serrapeptase and nattokinase are generally well tolerated in the short term, though long-term safety hasn't been thoroughly studied. Serrapeptase may cause epigastric pain, stomach upset, and nausea, though these side effects occur at the same rate as placebo in trials.

Nattokinase carries a rare but serious risk of bleeding, and case reports exist of brain hemorrhage when combined with aspirin. If you're sensitive to natto (fermented soybeans), nattokinase can trigger allergic reactions including anaphylaxis.
