The evidence supporting these ingredients is limited. For serrapeptase, the data we hold rates its use for chronic bronchitis, asthma, atherosclerosis, back pain, and osteoporosis as having insufficient reliable evidence.
Similarly, nattokinase's effectiveness for angina, atherosclerosis, cancer, cardiovascular disease, and chronic fatigue syndrome are all rated as insufficient reliable evidence. Neither ingredient has established proof in the studies we hold.

Both serrapeptase and nattokinase are generally well tolerated in the short term, though long-term safety hasn't been thoroughly studied. Serrapeptase may cause epigastric pain, stomach upset, and nausea, though these side effects occur at the same rate as placebo in trials.
Nattokinase carries a rare but serious risk of bleeding, and case reports exist of brain hemorrhage when combined with aspirin. If you're sensitive to natto (fermented soybeans), nattokinase can trigger allergic reactions including anaphylaxis.
During pregnancy, neither ingredient has enough safety data — both should be avoided. The same applies to breastfeeding: there isn't enough information to know they're safe, so it's best to skip them while nursing.

Nattokinase
Anticoagulant/Antiplatelet Drugs
Nattokinase might increase the risk of bleeding when used with anticoagulant/antiplatelet drugs.
Nattokinase has thrombolytic and anticoagulant effects. There is a case report of intracranial hemorrhage in a patient who added nattokinase to aspirin therapy.
Antihypertensive Drugs
Theoretically, nattokinase might increase the risk of hypotension when used with antihypertensive drugs.
Some clinical research suggests that nattokinase may reduce systolic and diastolic blood pressure.
Serrapeptase
Anticoagulant/Antiplatelet Drugs
Theoretically, taking serrapeptase with drugs that have antiplatelet or anticoagulant effects might increase the risk of bruising and bleeding.
In vitro research shows that serrapeptase has fibrinolytic activity.
