Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
NattoZyme contains one active ingredient: nattokinase NSK-SD, a fibrinolytic enzyme derived from natto (fermented soybeans). The softgel capsule also contains inactive ingredients — soybean oil, gelatin, glycerin, soybean lecithin, glycerin fatty acid ester, and beeswax — that help deliver the active ingredient.
Does it work?
Not established
The evidence we hold does not establish nattokinase as effective for any condition we checked — angina, atherosclerosis, cancer, cardiovascular disease, and chronic fatigue syndrome all show insufficient reliable evidence to rate. That doesn't mean it doesn't work; it means the clinical data available to us isn't strong enough to say one way or the other.
If you're considering this product for a specific health goal, talk with your doctor about what the current evidence supports.
How safe is it?
Well-documented data
Nattokinase is generally well tolerated short-term, but because it has blood-thinning effects, caution is warranted if you have any bleeding risk. Rare reports of bleeding have occurred, including a case of brain hemorrhage in someone combining nattokinase with aspirin.
Hypersensitivity reactions, including anaphylaxis, have been reported in people sensitive to natto or to components in it — symptoms can start within 2 hours and may involve the throat and larynx. There isn't enough safety data to recommend nattokinase during pregnancy, so it's best avoided then.
Safety while breastfeeding is unknown, so avoid it to be safe. If you're pregnant, planning to become pregnant, or breastfeeding, talk with your doctor or pharmacist before taking this product.
Meds to double-check
Moderate interaction found
Before you take NattoZyme, check with your doctor or pharmacist if you're on blood thinners (anticoagulants) or antiplatelet drugs like aspirin — nattokinase can increase bleeding risk, and a case of brain hemorrhage has been reported with the combination. Also double-check if you take blood pressure medications, as nattokinase may lower blood pressure further.
The bottom line
Scorecard at a glanceFully disclosed formula with no established evidence rating for its marketed use. Moderate medication interactions have been identified, and safety information is well characterized.
NattoZyme is a single-ingredient product that may appeal to people looking for a fibrinolytic enzyme, but the clinical evidence we hold doesn't yet establish it for any specific condition. The main concern is interaction with blood thinners and blood pressure medications — a combination that requires careful oversight from your doctor or pharmacist.
If you're on any anticoagulant or antihypertensive drug, or if you have a bleeding disorder or sensitivity to soy products, check with your healthcare provider before starting it.

NattoZyme Nattokinase 36 mg by NutriCology Drug Interactions
HelloPharmacist Interaction Report
NattoZyme Nattokinase 36 mg contains nattokinase, an enzyme that can interact with blood pressure medications and blood thinners.
The most serious concern is with blood thinners (anticoagulants) and antiplatelet drugs like aspirin — nattokinase has thrombolytic and anticoagulant effects, and there is a case report of brain bleeding (intracranial hemorrhage) in a patient who combined nattokinase with low-dose aspirin. This is a Moderate severity interaction.
Read the full breakdown — every affected drug type, severity by severity
Nattokinase may also increase the risk of low blood pressure (hypotension) when used alongside antihypertensive drugs. Some clinical research suggests nattokinase itself can lower both systolic and diastolic blood pressure, so the combination could potentially cause blood pressure to drop too far.
Additionally, we hold no data on how nattokinase might interact with other medication types. Altogether, these interactions span 289 individual medications.
Before you start this product, check your exact medications with the tool on this page or talk with your doctor or pharmacist — especially if you take any blood thinner or blood pressure medication.
