The data we have shows it's usually tolerated when taken by mouth or applied to skin, with the most common side effects being stomach pain and heartburn. Rarely, some people experience allergic reactions including anaphylaxis (severe whole-body reaction), throat closing, or migraines triggered by onion's smell.
Large doses of onion powder can cause stomach distress or heartburn, and one case reported esophageal spasm (muscle cramping in the food pipe) from raw onions. Because this is a concentrated supplement and not food, doses used in research are less well studied than everyday onion consumption.

If you take lithium for mood disorders, this extract's potential diuretic effect (increasing urine output) could raise lithium levels in your body, possibly requiring a dose adjustment. Drugs processed through a liver enzyme called CYP2E1 may reach higher levels in your blood, though this hasn't been proven in people yet, only in animal work.
Finally, if you have even a mild onion allergy, taking aspirin alongside this supplement could make allergy symptoms worse — one patient reported severe swelling and hives when combining the two.

Onion Bulb Extract, Dry
Anticoagulant/Antiplatelet Drugs
Theoretically, concomitant use of anticoagulant or antiplatelet drugs with onion might increase the risk of bleeding.
In vitro research shows that onion inhibits platelet aggregation.
Antidiabetes Drugs
Concomitant use of antidiabetes drugs with onion may increase the risk of hypoglycemia.
Animal research and clinical research shows that taking onion can lower blood glucose levels. Monitor blood glucose levels closely.
Aspirin
Concomitant use of aspirin with onion may worsen onion allergy.
In one case report, a patient with a mild onion allergy reported worsening allergy, including swelling and severe urticaria, after taking aspirin.
Cytochrome P450 2E1 (Cyp2E1) Substrates
Theoretically, taking onion might increase the levels and clinical effects of drugs metabolized by CYP2E1.
Animal research shows that taking onion powder inhibits CYP2E1. However, this interaction has not been reported in humans.
Lithium
Onion is thought to have diuretic properties. Theoretically, due to these potential diuretic effects, onion might reduce excretion and increase levels of lithium. The dose of lithium might need to be decreased.
