Here's the gist for your aspirin with codeine product. The aspirin part already makes your platelets (the cells that help blood clot) a bit slippery, so they don't stick together as easily. In lab dishes, onion compounds like quercetin and its sulfur compounds seem to nudge platelets in that same direction. So in theory, piling concentrated onion supplements on top of aspirin could add to the blood-thinning effect.

What that might look like in real life: easier bruising, nosebleeds, or bleeding that takes longer to stop. I'll be honest though, this is theoretical and based on lab work, not strong human studies. Onion as food on your plate is very unlikely to cause trouble. High-dose onion capsules are the question mark, so run those by your pharmacist or doctor first.

Here's the gist for your aspirin with codeine and onion. This isn't really about how your body breaks down the medicine. The concern, which comes from a single case report, is that aspirin may make an existing onion allergy worse. In that one report, a person with a mild onion allergy developed swelling and a bad case of hives (urticaria) after taking aspirin.

So if you have no onion allergy, eating onion in normal food amounts is very unlikely to cause a problem. The evidence here is anecdotal (one case), so I wouldn't lose sleep over it. But if you do react to onion, mention it to your pharmacist or doctor before pairing it with aspirin, and watch for hives, itching, or swelling.
