Aspirin, Codeine Phosphate, Phenobarbital falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Onion interacts with.

Theoretically, concomitant use of anticoagulant or antiplatelet drugs with onion might increase the risk of bleeding.
In vitro research shows that onion inhibits platelet aggregation.

This combination pill contains three medicines, but the part that interacts with onion is the aspirin. Aspirin already thins the blood by keeping platelets from clumping together. Lab studies suggest onion compounds (like quercetin and its sulfur compounds) may do something similar. So in theory, piling onion on top of aspirin could nudge your bleeding risk up a little, showing up as easier bruising, nosebleeds, or bleeding that takes longer to stop.

Keep in mind this is theoretical, based on test-tube work, not real patient studies. Onion in normal food amounts is very unlikely to cause trouble. The question mark is really about concentrated onion supplements. Before starting one, just check with your pharmacist or doctor, especially since this product also has codeine and phenobarbital on board.

Concomitant use of aspirin with onion may worsen onion allergy.
In one case report, a patient with a mild onion allergy reported worsening allergy, including swelling and severe urticaria, after taking aspirin.

This combination pill contains aspirin along with codeine and phenobarbital. The interaction we have on file is really about the aspirin part teaming up with onion. The evidence here is thin, just a single case report, so I don't want to alarm you. In that one report, a person who already had a mild onion allergy noticed it got worse after taking aspirin, with swelling and a serious case of hives (urticaria).

So this only matters if you actually have an onion allergy to begin with. Aspirin can make some allergic and hive-type reactions flare up more strongly. If you're allergic to onion, it's worth mentioning to your pharmacist or doctor before using this medicine. Everyday cooking amounts of onion are unlikely to be a concern for most folks.
