Pro-Flora Immune is a 5-ingredient capsule built around live probiotic bacteria and a supporting ingredient. The active probiotics are Lactobacillus plantarum Lp-115, Bifidobacterium lactis Bl-04, a proprietary probiotic blend, B. longum BB536, and L. acidophilus NCFM.
Lactoferrin, a protein found naturally in milk and other body fluids, rounds out the formula. The capsule also contains inactive ingredients — palm oil, fish gelatin, glycerin, soy lecithin, silicon dioxide, and pectin — that help hold the product together and protect the live cultures.
Does it work?

The evidence for what this probiotic blend can do is limited and mixed. L. acidophilus has possibly effective data for irritable bowel syndrome (IBS), Helicobacter pylori infection, antibiotic-associated diarrhea, and bacterial vaginosis.
B. longum BB536 has insufficient evidence to rate for cognitive decline, hay fever, antibiotic-associated diarrhea, anxiety, asthma, and athletic performance. Lactoferrin is possibly effective for sepsis and pregnancy-related iron deficiency, but likely ineffective for fetal and premature infant mortality and necrotizing enterocolitis.
For most of the conditions this blend might be marketed for, the evidence just isn't solid enough yet.
How safe is it?
Well-documented data
The probiotic strains here are generally well tolerated in healthy people, though caution is warranted if you're seriously ill or immune-compromised — live probiotics carry a rare but documented risk of infection in those populations. Gastrointestinal side effects like bloating, flatulence, and abdominal discomfort have been reported rarely with both L. acidophilus and B. longum.
Lactoferrin is well tolerated at typical doses, though higher doses (7.2 grams daily) may cause constipation, diarrhea, or fatigue; this product is unlikely to reach those levels. For pregnancy, the probiotics here are rated possibly safe, and lactoferrin is rated likely safe.
For breastfeeding, limited safety data exist for both the probiotics and lactoferrin, so talk with your doctor or pharmacist before use.

B. longum BB536
Antibiotic Drugs
Theoretically, taking Bifidobacterium longum with antibiotic drugs might decrease the effectiveness of B. longum.
Since B. longum preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. longum preparations by at least 2 hours.
L. acidophilus NCFM
Antibiotic Drugs
Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
