Both Lactobacillus acidophilus and Bifidobacterium bifidum are generally well tolerated in healthy adults and children. That said, the data come with cautions: check with your doctor before use, especially if you have a weakened immune system.

Uncommon side effects from L. acidophilus include stomach discomfort, abdominal pain, indigestion, gas, bloating, diarrhea, vomiting, and burping. A small number of trial participants taking a probiotic blend containing L. acidophilus reported rash and itching, though it's unclear whether the probiotic was responsible.

Rare but serious infections (bacteremia, sepsis, abscess, endocarditis) have been reported with L. acidophilus in some patients, though most appear linked to bacteria spreading from elsewhere in the body. Bloating and gas can occur with probiotics in general.

A single case of vomiting and fever was reported in a child taking B. bifidum with L. acidophilus, but causality is uncertain. For pregnancy, L. acidophilus is rated possibly safe; B. bifidum is also possibly safe in pregnancy.

Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
