Lactobacillus acidophilus shows possibly effective ratings for irritable bowel syndrome (IBS), Helicobacter pylori infection, antibiotic-associated diarrhea, and bacterial vaginosis; it's possibly ineffective for atopic disease and has insufficient evidence for acute respiratory distress syndrome.
The other checked strains—Bifidobacterium longum, Bifidobacterium breve, and Lactobacillus bulgaricus—each have insufficient evidence for most conditions tested, with Bifidobacterium breve rated possibly ineffective for age-related cognitive decline and sepsis. Many of the effectiveness claims remain unsettled in the research we have on file.
How safe is it?

These probiotics are generally well tolerated in healthy adults and children. Both Lactobacillus acidophilus and Bifidobacterium longum carry a caution for people who are seriously ill or immune-compromised, since rare cases of bacterial infection (bacteremia, sepsis) have been reported in critically ill patients and preterm infants.
Mild gastrointestinal side effects—bloating, flatulence, abdominal discomfort, and diarrhea—are uncommon but have occurred. Rare cases of rash and itching were reported in one trial, though it's unclear whether they came from the probiotics themselves.

Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
