Pentamidine falls under ANTIBIOTIC DRUGS — the group Bifidobacterium Breve interacts with.

Theoretically, taking Bifidobacterium breve with antibiotic drugs might decrease the effectiveness of B. breve.
Since B. breve preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. breve preparations by at least 2 hours.

Here's the simple version: this interaction goes one direction only, and it works against your probiotic, not your medication. Pentamidine won't be weakened or made stronger by Bifidobacterium breve. Instead, since pentamidine fights germs, it can also kill off the live, friendly bacteria in your B. breve supplement, leaving you with less benefit from the probiotic.

One thing worth knowing: pentamidine for pneumocystis pneumonia is usually given as an injection (or sometimes inhaled), so it doesn't sit in your gut the same way an oral antibiotic would, which makes this less of a concern in practice. This is a theoretical concern based on how these things behave, not strong proof. If you want full value from the probiotic, your pharmacist can help you time the two well apart.

Pentam 300 (pentamidine isethionate for injection) is indicated for the treatment of pneumonia caused by Pneumocystis carinii (PCP), a potentially life-threatening lung infection that most commonly affects people with weakened immune systems.

Because of its serious risks, Pentam 300 should only be given to patients in whom Pneumocystis carinii infection has actually been confirmed.
