Here is the interaction we found between these two, with the evidence behind it.
ANTIBIOTIC DRUGS
Why this applies to Amikacin
Amikacin falls under ANTIBIOTIC DRUGS — the group Bifidobacterium Breve interacts with.
Since B. breve preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. breve preparations by at least 2 hours.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the simple version: this interaction runs the other way from what people usually worry about. Amikacin is a strong antibiotic, and its job is to kill bacteria. The catch is that a probiotic like Bifidobacterium breve is bacteria, just the friendly kind. If you swallow your probiotic right around the same time you're getting amikacin, the antibiotic can wipe out a good chunk of those live organisms before they ever do any good.
So the worry isn't that your amikacin becomes too strong or too weak. Amikacin keeps working just fine. It's the probiotic that loses its punch. A common tip is to space them apart by at least a couple of hours. This is theoretical, based on how the two work, so check with your pharmacist or doctor on timing for your situation.

From our pharmacist’s overview on the Amikacin page
“The supplements that deserve genuine wariness with amikacin (Amikin, Arikayce) are the ones that touch the kidneys. Amikacin is a potent antibiotic that can strain kidney function on its own, so herbs like Siberian cocklebur and asarabacca (often contaminated with aristolochic acid, a known kidney toxin) should stay off the table durin…”

About Bifidobacterium Breve
What is Bifidobacterium Breve?
- Digestive health and regularity
- Infant colic and gut support
- Diarrhea (including antibiotic-related)
- Constipation
- General immune and gut microbiome support
Bifidobacterium breve is a 'friendly' gut bacterium taken as a probiotic, most often to support digestion and balance the gut microbiome. Some studies suggest possible benefits for certain digestive and infant conditions, but evidence is still limited and varies by strain. It is generally well tolerated in healthy people, but anyone with a weak immune system or serious illness should check with a doctor or pharmacist first.
About Amikacin Oral Inhalation
What Is Amikacin Oral Inhalation Used For?
Brand names: Amikin, Arikayce
Arikayce is approved for adults with refractory Mycobacterium avium complex (MAC) lung disease — a serious bacterial lung infection that has not responded to at least 6 consecutive months of a standard multidrug antibiotic regimen. It is used as part of a combination treatment plan, not on its own.
This approval is limited: Arikayce is intended only for adults who have few or no other treatment options. It is not recommended for patients whose MAC lung disease has not yet failed standard therapy.
