Probiotic Pearls Advantage contains 4 active ingredients: two strains of Bifidobacterium (B. longum BB536 and Bifidobacterium lactis Bl-04), Lactobacillus plantarum Lp-115, and Lactobacillus acidophilus (the HOWARU strain). These are live beneficial bacteria meant to support your gut health.
The capsule also contains inactive ingredients — palm oil, fish gelatin, glycerin, sunflower lecithin, and pectin — which help hold and deliver the probiotics.
Does it work?
Moderate evidence
The evidence for what this product does is mixed and incomplete. The Lactobacillus acidophilus strain has possibly effective evidence for irritable bowel syndrome, Helicobacter pylori infection, antibiotic-associated diarrhea, and bacterial vaginosis.
B. longum BB536 and Lactobacillus plantarum haven't shown enough reliable evidence for any condition in the data we hold. For several other uses — like age-related cognitive decline, asthma, and athletic performance — the evidence is simply insufficient to rate.
How safe is it?
Well-documented data
Both probiotic strains documented here are generally well tolerated in healthy adults and children, though data are limited. Rare gastrointestinal side effects with B. longum BB536 include abdominal discomfort, flatulence, constipation, and one reported case of dry cough.
Lactobacillus acidophilus may rarely cause similar stomach symptoms — abdominal pain, bloating, diarrhea — though these are uncommon when combined with other probiotics. Live probiotics carry a theoretical risk of infection in people who are seriously ill or immune-compromised; B. longum bacteremia and L. acidophilus sepsis have been reported in critically ill patients and preterm infants.
For pregnancy and breastfeeding, B. longum BB536 is rated possibly safe, but we hold no safety data for the other three ingredients in this product, so talk with your doctor or pharmacist before use if you're pregnant or nursing.

B. longum BB536
Antibiotic Drugs
Theoretically, taking Bifidobacterium longum with antibiotic drugs might decrease the effectiveness of B. longum.
Since B. longum preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. longum preparations by at least 2 hours.

Antibiotic Drugs
Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
