This product contains 4 active ingredients. Vitamin B-12 supports nerve and blood cell health and is used when your body can't absorb it well from food or has a deficiency.
Vitamin B-6 helps your body use amino acids and create neurotransmitters; it's important for brain function and immune health. Folate (the 5-MTHF form) is a B vitamin your body uses for DNA synthesis and cell division, and this form is already in the active shape your body uses.
SAMe (S-adenosylmethionine) is a compound your cells make naturally and use in hundreds of chemical reactions, particularly those involving mood, joint health, and liver function. The inactive ingredients are microcrystalline cellulose, vegetable stearate, calcium chloride anhydrous, and calcium oxide—these are fillers and capsule materials.
Does it work?
Strong evidence
Vitamin B-12 is effective for treating B-12 deficiency, a condition called Imerslund-Grasbeck disease, and cyanide poisoning; it's also possibly effective for canker sores and nerve pain after shingles. Vitamin B-6 is effective for sideroblastic anemia (a blood disorder), B-6 deficiency, and a seizure condition called pyridoxine-dependent epilepsy; it's likely effective for lowering homocysteine (a risk marker) and possibly effective for pregnancy-related nausea and vomiting.
SAMe is likely effective for osteoarthritis and possibly effective for depression and cholestasis (bile duct disease). For Alzheimer disease and ADHD, the evidence we hold on file is insufficient to rate whether it works.
Folate's effectiveness data is not on file with us.
How safe is it?
Well-documented data
Vitamin B-12 is generally very safe—your body removes excess in the urine, and there's no established upper limit. It's safe at normal recommended amounts during pregnancy and breastfeeding.
Vitamin B-6 is well tolerated at doses under 100 mg daily, but high doses over time can damage nerves (sensory neuropathy). It's used during pregnancy for morning sickness but only under a doctor's guidance, and normal dietary amounts are fine during breastfeeding.
SAMe is often well tolerated short-term but can cause stomach upset (nausea, constipation, diarrhea, heartburn), dizziness, headache, insomnia, and nervousness, especially at higher doses. It may trigger mania in people with bipolar disorder.
Safety during pregnancy is possibly safe, but safety during breastfeeding is not well established. The safety profile of folate is not on file with us.

The most serious interaction documented is with serotonergic drugs (antidepressants, certain pain medications, and other serotonin-boosting medications) and SAMe—combining them raises the risk of serotonin syndrome, a potentially serious condition marked by agitation, confusion, rapid heartbeat, and high blood pressure.

Vitamin B-6 can interact with several drug categories. It may reduce blood pressure additively with antihypertensive medications, increase photosensitivity caused by amiodarone (a heart rhythm drug), and at high doses may reduce the effectiveness of the seizure medications phenobarbital and phenytoin.
Vitamin B-6 may also speed up the breakdown of levodopa (used for Parkinson disease), though this doesn't happen if you're taking it with carbidopa.
Vitamin B-12 has a minor interaction with metformin, a common diabetes medication—metformin can lower B-12 levels over time by interfering with its absorption. SAMe may also reduce how well levodopa works by metabolizing it in your body.

Vitamin B-6
Amiodarone (Cordarone)
Theoretically, vitamin B6 might increase the photosensitivity caused by amiodarone.
Despite initial case reports suggesting that pyridoxine may have a protective effect against amiodarone-induced photosensitivity, preliminary clinical research suggests that pyridoxine may actually exacerbate this adverse effect.
Antihypertensive Drugs
Theoretically, vitamin B6 may have additive effects when used with antihypertensive drugs.
Research in hypertensive rats shows that vitamin B6 can decrease systolic blood pressure. Similarly, clinical research in patients with hypertension shows that taking high doses of vitamin B6 may reduce systolic and diastolic blood pressure, possibly by reducing plasma levels of epinephrine and norepinephrine.
Phenobarbital (Luminal)
High doses of vitamin B6 may reduce the levels and clinical effects of phenobarbital.
Preliminary clinical evidence suggests that vitamin B6 200 mg daily can reduce plasma levels of phenobarbital, possibly by increasing metabolism. It is not known whether lower doses have any effect. Advise people taking phenobarbital to avoid high doses of vitamin B6.
Phenytoin (Dilantin)
High doses of vitamin B6 may reduce the levels and clinical effects of phenytoin.
Preliminary clinical evidence suggests that vitamin B6 200 mg daily can reduce plasma levels of phenytoin, possibly by increasing metabolism. It is not known whether lower doses have any effect. Advise people taking phenytoin to avoid high doses of vitamin B6.
Levodopa
Vitamin B6 may increase the metabolism of levodopa when taken alone, but not when taken in conjunction with carbidopa.
Vitamin B6 (pyridoxine) enhances the metabolism of levodopa, reducing its clinical effects. However, this interaction does not occur when carbidopa is used concurrently with levodopa (Sinemet). Therefore, it is not likely to be a problem in most people.
SAMe
Levodopa
SAMe might reduce the effectiveness of levodopa.
SAMe methylates levodopa, which might reduce its effectiveness for treating Parkinson disease.
Serotonergic Drugs
Taking SAMe with serotonergic drugs might increase the risk of serotonin syndrome and other serotonergic side effects.
SAMe has serotonergic effects. Theoretically, combining serotonergic drugs with SAMe might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders. In one case report, SAMe 100 mg intramuscularly was given daily with clomipramine (Anafranil) 25 mg per day. When the clomipramine dose was increased to 75 mg per day the patient experienced serotonin syndrome about 48-72 hours later, requiring hospitalization.
Vitamin B12
Metformin (Glucophage)
Metformin, a common medication used to manage type 2 diabetes, has been associated with lower vitamin B12 levels in some individuals. Prolonged use of metformin can interfere with the absorption of B12 in the digestive system, potentially leading to a deficiency in this essential vitamin.
