What’s inside
Full disclosure
Sang Zhi by Herbal Terra is a single-ingredient liquid extract made from certified organic Sang Zhi, the dried twig of white mulberry (Morus alba). White mulberry twig has a long history in herbal practice, though the supplement form is more concentrated than the food form you might encounter in tea or culinary use.
Does it work?
Couldn't assess
The evidence for white mulberry isn't established in our data for any of the conditions it's marketed for — including hair loss (androgenic alopecia), bone health, cold symptoms, blood sugar control, and cholesterol levels. Each of these carries a rating of Insufficient Reliable Evidence To Rate, which means the clinical research backing these uses is either too limited or not yet strong enough to draw firm conclusions.
How safe is it?
Well-documented data
White mulberry leaf appears to be generally well tolerated when taken by mouth. In a clinical trial, the most common side effects were temporary: bloating and flatulence occurred in about half the participants, loose stools in a quarter, and constipation in about a fifth — but these tended to ease as the study went on, suggesting they may be temporary for some people.
The product label cautions that while white mulberry leaf is likely safe as a food, concentrated supplements are less studied and may lower blood sugar. There isn't enough safety data to recommend it during pregnancy or while breastfeeding; talk with your doctor or pharmacist about whether this product is right for you in those situations.
Meds to double-check
Moderate interaction found
Before taking Sang Zhi, check with your doctor or pharmacist if you're on antidiabetes drugs (including metformin) or any medications that are OCT2 substrates. White mulberry can lower blood sugar and may slow how your kidneys clear certain drugs, both Moderate severity concerns.
If you're on diabetes medication, you'll need close glucose monitoring if you add this product.
The bottom line
Scorecard at a glanceFully disclosed formula with no assessable stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
Sang Zhi is a single white mulberry extract that's generally well tolerated but carries real interactions with diabetes medications and a broad class of other drugs. The evidence for what it actually does isn't strong enough to rate for any of its claimed uses.
If you take any medication — especially diabetes drugs — run it through the checker on this page and talk with your doctor or pharmacist before starting.

Sang Zhi by Herbal Terra Drug Interactions
HelloPharmacist Interaction Report
Sang Zhi by Herbal Terra contains white mulberry leaf extract, which has documented interactions with two major drug categories.
The most serious concern is with antidiabetes drugs — white mulberry can lower blood sugar levels, which means it could increase your risk of low blood sugar (hypoglycemia) if you're already taking diabetes medication. This is a Moderate severity interaction, and it means you'll need to monitor your glucose closely if you combine them.
Read the full breakdown — every affected drug type, severity by severity
White mulberry also interacts with a group of drugs called OCT2 substrates (Organic Cation Transporter 2 substrates) with Moderate severity. Animal research shows the extract can slow how your kidneys clear these medications, potentially raising their levels in your body and causing more side effects.
Metformin, a common diabetes drug, is an OCT2 substrate — so if you're on metformin, this concern applies on top of the blood-sugar effect.
Altogether, these interactions span 87 individual medications. Before you start Sang Zhi, check your exact medications — especially any for diabetes or any that your doctor or pharmacist flags as OCT2 substrates — using the medication checker on this page.

Antidiabetes Drugs
Theoretically, white mulberry leaf might increase the risk of hypoglycemia when taken with antidiabetes drugs.
Clinical research shows that white mulberry leaf can lower blood glucose levels. Advise patients to monitor glucose levels closely.
Organic Cation Transporter 2 (Oct2) Substrates
Theoretically, white mulberry leaf might slow the elimination and increase the adverse effects of drugs which are OCT2 substrates.
Animal research shows that coadministration of white mulberry leaf extract with metformin, an OCT2 substrate, slows the renal elimination of metformin via inhibition of OCT2 activity. OCT2 is expressed in the kidneys and is responsible for transporting cationic drugs into tubular epithelial cells in order to be excreted in the urine.
