Does it work?
Strong evidence
Pygeum africanum bark extract is likely effective for benign prostatic hyperplasia (BPH), a common prostate condition. However, evidence for other uses listed—such as Alzheimer disease, malaria, sexual desire, and psychosis—is insufficient.
Saw palmetto is possibly ineffective for BPH, though it may be possibly effective for transurethral resection of the prostate (TURP). Evidence for its use in hair loss, chronic bronchitis, and lower urinary tract symptoms isn't established in the data we hold.
Nettle root extract is possibly effective for diabetes; evidence for hay fever, anemia, asthma, prostate health, and gum health is insufficient.
How safe is it?
Well-documented data
Pygeum africanum bark extract and saw palmetto are both generally well tolerated in the short term, though saw palmetto's long-term safety is not well studied. Saw palmetto's most common side effects are mild and reversible—including abdominal pain, constipation, diarrhea, dizziness, nausea, and fatigue.
Nettle root extract is generally well tolerated; the most common effects are constipation and diarrhea. Rare case reports of serious liver injury exist for nettle.
Both pygeum and saw palmetto carry safety concerns in pregnancy and lactation—the data rates saw palmetto as likely unsafe, and both pygeum and nettle root as best avoided. Nettle root data does not provide a lactation rating; talk with your doctor or pharmacist if you are breastfeeding.

Animal research suggests that stinging nettle has diuretic and natriuretic properties, which could alter the excretion of lithium. The dose of lithium might need to be decreased.
Warfarin (Coumadin)
There is some concern that stinging nettle might decrease the effects of anticoagulant drugs such as warfarin.
Stinging nettle contains a significant amount of vitamin K. When taken in large quantities, this might interfere with the activity of warfarin.
