SmartBRAIN contains 3 active ingredients designed to support brain function and cognition. Acetyl-L-carnitine is an amino acid derivative that may help with age-related cognitive decline, Alzheimer disease, alcohol use disorder, diabetic nerve damage, and depression.
Alpha-GPC (alpha-glycerophosphatidylcholine) is a choline compound that appears to support Alzheimer disease symptoms. Phosphatidylserine (marketed here as Sharp-PS) is a phospholipid found in cell membranes and may help with age-related cognitive decline and Alzheimer disease.
The capsules also contain inactive ingredients—glycerin, cellulose, vitamin E acetate, titanium dioxide, and silicon dioxide—which serve as binders, thickeners, and capsule materials.
Does it work?
Moderate evidence
Acetyl-L-carnitine is rated possibly effective for age-related cognitive decline, Alzheimer disease, alcohol use disorder, diabetic neuropathy, and depression based on available research. Alpha-GPC is rated possibly effective for Alzheimer disease but has insufficient evidence for brain tumor, cognitive function, vascular dementia, and stroke.
Phosphatidylserine (Sharp-PS) is rated possibly effective for age-related cognitive decline and Alzheimer disease, with insufficient evidence for athletic performance, ADHD, cognitive function, and dementia. In short, the strongest support exists for Alzheimer disease and age-related cognitive decline across all three ingredients, though evidence quality varies.
How safe is it?
Well-documented data
Acetyl-L-carnitine is generally well tolerated short-term, but long-term safety is not fully established. Common side effects include agitation, dry mouth, headache, insomnia, and reduced appetite.
Some people report a fishy odor in urine, breath, or sweat. Gastrointestinal symptoms like nausea, vomiting, diarrhea, and constipation can occur but are not significantly more common than with placebo.
Alpha-GPC and phosphatidylserine are generally well tolerated in studies, though long-term safety is not established for either. Alpha-GPC rarely causes dizziness, headache, insomnia, and gastrointestinal upset, and has been rarely associated with stroke with use of at least 2 months.
Phosphatidylserine may cause flatulence, gastrointestinal upset, headache, insomnia, and nausea—insomnia is more likely at higher doses (600 mg). There is not enough safety information during pregnancy for any of these ingredients; avoid use unless your doctor advises otherwise.
Safety while breastfeeding is unknown for acetyl-L-carnitine and phosphatidylserine; avoiding them is the safer choice. Safety information for alpha-GPC during breastfeeding is not on file.

Also confirm any blood thinners like warfarin or acenocoumarol, thyroid hormone replacement, anticholinergic drugs (such as those for urinary or neurological conditions), or cholinergic drugs (such as for myasthenia gravis or Alzheimer disease).
Alpha-GPC may also reduce the effectiveness of scopolamine motion sickness patches.

The most serious documented interaction is acetyl-L-carnitine with serotonergic drugs (antidepressants, certain pain relievers, migraine medications), which carries a Moderate severity risk of serotonin syndrome and related cerebral blood vessel effects. Acetyl-L-carnitine also has Moderate interactions with blood thinners (warfarin and acenocoumarol) by potentially boosting their anticoagulant effect, and with thyroid hormone replacement by possibly reducing its effectiveness.

Alpha-GPC carries a Minor interaction with scopolamine patches (motion sickness medication), theoretically reducing their effectiveness. Phosphatidylserine (Sharp-PS) has Moderate interactions with both anticholinergic drugs (used for certain neurological and urinary conditions) and cholinergic drugs (used for myasthenia gravis and Alzheimer disease), potentially weakening or amplifying their effects respectively.

Sharp-PS
Anticholinergic Drugs
Theoretically, phosphatidylserine might decrease the effectiveness anticholinergic drugs.
Phosphatidylserine is thought to increase acetylcholine levels, which could theoretically interfere with the activity of anticholinergic agents.
Cholinergic Drugs
Theoretically, phosphatidylserine might have additive effects with cholinergic drugs.
Phosphatidylserine is thought to increase acetylcholine levels, which could theoretically lead to additive cholinergic effects when used with cholinergic drugs.
Acetyl L-Carnitine Hydrochloride
Acenocoumarol (Sintrom)
Theoretically, acetyl-L-carnitine might increase the anticoagulant effects of acenocoumarol.
L-carnitine, the parent compound of acetyl-L-carnitine, might enhance the anticoagulant effects of acenocoumarol, an oral anticoagulant that is similar to warfarin, but shorter-acting. There are at least two case reports of INR elevation when L-carnitine was taken with acenocoumarol. In one case, a 33-year-old male with a previously stable INR had an elevated INR of 4.65 after L-carnitine was started and continued for 10 weeks. INR normalized after discontinuation of the L-carnitine-containing product. It is unclear if such an interaction would also occur with acetyl-L-carnitine.
Serotonergic Drugs
Theoretically, acetyl-L-carnitine might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders, when taken with serotonergic drugs.
Animal research shows that acetyl-L-carnitine can increase levels of serotonin in the brain.
Thyroid Hormone
Theoretically, acetyl-L-carnitine might decrease the effectiveness of thyroid hormone replacement.
L-carnitine appears to act as a peripheral thyroid hormone antagonist by inhibiting entry of thyroid hormone into the nucleus of cells. Taking L-carnitine also seems to diminish some of the symptoms of hyperthyroidism. It is unclear if such an interaction would occur with acetyl-L-carnitine.
Warfarin (Coumadin)
Theoretically, acetyl-L-carnitine might increase the anticoagulant effects of warfarin.
L-carnitine, the parent compound of acetyl-L-carnitine, might increase the anticoagulant effects of acenocoumarol, a shorter-acting oral anticoagulant similar to warfarin. There is not enough information to know whether this interaction occurs with acetyl-L-carnitine and warfarin.
Alpha-Glycerophosphatidylcholine
Scopolamine (Transderm Scop)
Theoretically, alpha-GPC might decrease the effects of scopolamine.
A small clinical study shows that alpha-GPC can partially counteract the attention and memory impairment effects caused by scopolamine given intramuscularly. Whether alpha-GPC can decrease the beneficial anti-motion sickness effects of the scopolamine patch (Transderm Scop) is unclear.
