For Lactobacillus acidophilus, the evidence shows it is possibly effective for irritable bowel syndrome (IBS), infection with Helicobacter pylori bacteria, antibiotic-associated diarrhea, and bacterial vaginosis. Lactobacillus acidophilus is generally well tolerated in healthy adults and children. Probiotic combinations may occasionally cause mild gastrointestinal side effects—stomach discomfort, bloating, gas, or diarrhea—but these are uncommon.

Rarely, rash or itching has been reported. Since L. acidophilus preparations contain live organisms, there is a small theoretical risk of infection in certain patients; most documented cases occurred in people with serious underlying illness or in specific circumstances.

Bifidobacterium longum is generally well tolerated in healthy people but requires caution in those who are seriously ill or have a weakened immune system. Rarely, mild abdominal discomfort, gas, or constipation may occur.

Cases of Bifidobacterium infection have been reported primarily in critically ill infants or premature infants. The product is rated possibly safe in pregnancy and lactation, though data are limited—ask your doctor before using while breastfeeding.

Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
