Lactobacillus acidophilus and Bifidobacterium bifidum are generally well tolerated in healthy adults and children, though people with weakened immune systems should be cautious. Bifidobacterium longum is also generally well tolerated in healthy people but requires caution in those who are seriously ill or immune-compromised.

Gastrointestinal side effects — bloating, flatulence, abdominal discomfort, and rarely diarrhea or vomiting — can occur, though they're uncommon. Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
