The remaining ingredients are inactive — cellulose, mannitol, citric acid, stearic acid, and several others that bind, fill, or coat the tablet.
Does it work?
Strong evidence
SAMe is likely effective for osteoarthritis, meaning there's solid evidence it can help with joint pain and function. For depression, the evidence is weaker — it's possibly effective, so it may help some people, but the research isn't conclusive.
For liver disease (cholestasis), it's also possibly effective. For Alzheimer disease and ADHD, there isn't enough reliable evidence in our data to say whether it works.
How safe is it?
Well-documented data
SAMe is generally well tolerated at standard doses, but side effects do happen — the most common are nausea, diarrhea or constipation, headache, dizziness, and insomnia. Higher doses make side effects more likely.
One important caution: SAMe may trigger or worsen mania if you have bipolar disorder, so talk with your doctor before taking it if that applies to you. During pregnancy, SAMe is rated possibly safe, but lactation data isn't on file — either way, discuss it with your doctor or pharmacist before use if you're pregnant or breastfeeding.

S-Adenosyl Methionine
Levodopa
SAMe might reduce the effectiveness of levodopa.
SAMe methylates levodopa, which might reduce its effectiveness for treating Parkinson disease.
Serotonergic Drugs
Taking SAMe with serotonergic drugs might increase the risk of serotonin syndrome and other serotonergic side effects.
SAMe has serotonergic effects. Theoretically, combining serotonergic drugs with SAMe might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders. In one case report, SAMe 100 mg intramuscularly was given daily with clomipramine (Anafranil) 25 mg per day. When the clomipramine dose was increased to 75 mg per day the patient experienced serotonin syndrome about 48-72 hours later, requiring hospitalization.
