Lemon balm is generally well tolerated for short-term use in healthy adults, though dizziness and sedation have been reported — though they don't occur more often than with placebo. A single case of heart rhythm changes (complete atrioventricular block and QT prolongation) was reported in a young woman using a different product containing lemon balm and other ingredients; it resolved when she stopped.

Motherwort is generally well tolerated orally but may cause diarrhea and stomach irritation. High doses (more than 3 grams) can trigger uterine bleeding.

Cactus grandiflorus may cause mouth burning, nausea, vomiting, and diarrhea. Bugleweed carries no reported adverse effects in our data.

During pregnancy: lemon balm should be avoided unless your doctor advises otherwise; motherwort and bugleweed are likely unsafe; cactus grandiflorus is best avoided. While breastfeeding: lemon balm requires caution and a conversation with your doctor; bugleweed should be avoided because it may reduce milk production; cactus grandiflorus is best avoided.

Cns Depressants

Theoretically, concomitant use of lemon balm might have additive effects with CNS depressant drugs.
Lemon balm seems to have CNS depressant activity in animals and in humans.

Theoretically, lemon balm might interfere with thyroid hormone replacement therapy.
In vitro, constituents of lemon balm extract bind to thyroid stimulating hormone (TSH), preventing TSH receptor-binding and leading to the inhibition of TSH-stimulated adenylate cyclase activity. In animals, lemon balm extract has been shown to decrease levels of circulating TSH and inhibit thyroid secretion.
