The evidence for these mushroom extracts is thin. For maitake mushroom, there's insufficient reliable evidence for hay fever, chemotherapy-induced anemia, laryngeal cancer, and chemotherapy-induced diarrhea.
Reishi mushroom shows possibly ineffective evidence for high cholesterol (hyperlipidemia), and insufficient evidence for altitude sickness, Alzheimer's disease, and athletic performance. Shiitake mushroom is possibly ineffective for dental plaque and has insufficient evidence for chemotherapy-induced anemia, atherosclerosis, breast cancer, and chemotherapy-induced nausea and vomiting.

How safe is it?
Well-documented data
Orally, all three mushroom extracts are generally well tolerated as food, but concentrated supplement safety hasn't been fully studied. The most common side effects reported in clinical trials are gastrointestinal — diarrhea, nausea, stomach upset, and abdominal discomfort.
Reishi may cause dizziness, dry mouth, itching, or rash; in rare cases, insomnia and headache. Shiitake as a cooked food is generally safe, but concentrated supplements carry more caution because data is limited.
Raw shiitake can cause shiitake dermatitis, a distinctive skin reaction. Both maitake and reishi should be avoided during pregnancy and breastfeeding because there isn't enough safety data.

Puritan's Pride Triple Mushroom Complex contains three mushroom extracts, each with documented interactions with common medications.
The most serious concern is maitake mushroom and warfarin (Coumadin), a blood thinner — there's limited evidence that maitake may increase warfarin's blood-thinning effects, and a case report showed one patient's INR (a measure of how thin the blood is) jumped to an unsafe level after taking maitake. This is a Moderate severity interaction.

Maitake and reishi can both lower blood pressure and blood sugar, so combining them with blood pressure medications (antihypertensive drugs) or diabetes medications carries a Moderate risk of dropping these too far — causing dizziness or low blood sugar (hypoglycemia). Reishi at higher doses may also increase bleeding risk if you're on blood thinners (anticoagulant or antiplatelet drugs) like aspirin or other anticoagulants beyond warfarin.
Shiitake mushroom can theoretically reduce how well your body metabolizes certain medications broken down by the enzyme CYP2D6, and it may interfere with immunosuppressant drugs (medications that weaken the immune system on purpose). Both are Moderate severity interactions, though the CYP2D6 effect hasn't been confirmed in humans yet.

organic Red Reishi Mushroom extract
Anticoagulant/Antiplatelet Drugs
Theoretically, high doses of reishi mushroom might increase the risk of bleeding.
A dose of 1.5 grams daily of reishi mushroom does not seem to decrease platelet aggregation, but a higher dose of 3 grams daily does.
Antidiabetes Drugs
Theoretically, reishi mushroom might have additive effects with antidiabetes drugs.
Animal research suggests that reishi mushroom decreases blood sugar. However, in patients with type 2 diabetes, taking reishi mushroom does not reduce fasting glucose levels, and its effects on glycated hemoglobin are inconsistent.
Antihypertensive Drugs
Theoretically, concurrent use of reishi mushroom with antihypertensive drugs might increase the risk of hypotension.
Reishi mushroom has shown hypotensive activity in animal research. Clinical evidence suggests that reishi mushroom reduces blood pressure in some, but not all, patients with hypertension.
organic Shiitake Mushroom extract
Cytochrome P450 2D6 (Cyp2D6) Substrates
Theoretically, shiitake mushroom might decrease levels of drugs metabolized by CYP2D6.
In vitro studies suggest that the shiitake mushroom extract AHCC might induce the CYP2D6 enzyme. This effect has not been reported in humans.
Immunosuppressants
Theoretically, taking shiitake mushroom might decrease the effects of immunosuppressive therapy.
In vitro evidence suggests that shiitake mushroom extracts stimulate immune function.
Maitake Mushroom extract
Antidiabetes Drugs
Theoretically, combining maitake mushroom with antidiabetes drugs might increase the risk of hypoglycemia.
Clinical research shows that taking maitake mushroom polysaccharide (MMP) can lower blood glucose levels in patients with types 2 diabetes.
Antihypertensive Drugs
Theoretically, combining maitake mushroom with antihypertensive drugs might increase the risk of hypotension.
Animal research shows that maitake mushroom can lower blood pressure.
Warfarin (Coumadin)
There is limited evidence that maitake mushroom may increase the anticoagulant effects of warfarin.
In a case report, a patient previously stabilized on warfarin developed an elevated international normalized ratio (INR) of 5.1 after taking maitake mushroom (Grifron-Pro Maitake D-Fraction) 1 drop/kg daily in three divided doses for one week. The elevated INR resolved after holding warfarin for two days, then reducing the dose by 11%. It is thought that the beta-glucan constituent of maitake mushroom might cause warfarin dissociation from proteins, resulting in increased free warfarin levels and increased warfarin effects.
