Theoretically, reishi mushroom might have additive effects with antidiabetes drugs.
Animal research suggests that reishi mushroom decreases blood sugar. However, in patients with type 2 diabetes, taking reishi mushroom does not reduce fasting glucose levels, and its effects on glycated hemoglobin are inconsistent.

Here's the situation with reishi and your alogliptin (Nesina). Both are thought to nudge blood sugar in the same downward direction, so in theory taking them together could lower your numbers a bit more than alogliptin alone. If that happened, you might notice signs of low blood sugar like shakiness, sweating, dizziness, or feeling unusually hungry or tired.

The honest truth, though, is that this concern rests mostly on animal studies. In people with type 2 diabetes, reishi has not reliably lowered fasting glucose, and its effect on A1C has been all over the map. So the real-world risk looks small. Still, if you decide to try reishi, it's smart to keep an eye on your home glucose readings and let your pharmacist or doctor know before starting.
