Theoretically, concurrent use of reishi mushroom with antihypertensive drugs might increase the risk of hypotension.
Reishi mushroom has shown hypotensive activity in animal research. Clinical evidence suggests that reishi mushroom reduces blood pressure in some, but not all, patients with hypertension.

Here's the gist: Iloprost (Ventavis) already works by relaxing and widening your blood vessels to ease the pressure in your lungs. Reishi mushroom may, on its own, nudge blood pressure down a little too. Put the two together and the worry is mostly additive, meaning your blood pressure could dip lower than expected. That might leave you feeling lightheaded, dizzy, flushed, or faint, especially when you stand up quickly.

I want to be honest about the evidence though: this is mostly based on animal studies and a few mixed human reports, so it's more theoretical than proven. Reishi isn't activated by Iloprost or anything like that. Still, if you're thinking about starting reishi, run it past your pharmacist or doctor first so they can keep an eye on your readings.

A dose of 1.5 grams daily of reishi mushroom does not seem to decrease platelet aggregation, but a higher dose of 3 grams daily does.

Here's the thing to keep in mind with iloprost (Ventavis): besides relaxing your lung blood vessels, it also makes platelets a little less sticky, so it can mildly raise bleeding risk on its own. Reishi may do something similar at high doses. In lab and animal studies, the concern is that 3 grams a day or more of reishi might slow platelet clumping, while a smaller 1.5 gram dose didn't seem to. Stack that on top of iloprost and the two could add up.

This is theoretical and based only on early studies, so don't panic. But watch for easy bruising, nosebleeds, bleeding gums, or cuts that take a while to stop. Please run your reishi plans past your pharmacist or doctor before starting.

Aurlumyn (iloprost injection) is FDA-approved to treat severe frostbite in adults. Specifically, it is used to reduce the risk of losing fingers or toes (digit amputation) after serious frostbite injury. The clinical evidence supporting this use comes from young, healthy adults who suffered frostbite at high altitudes.
