HelloPharmacist Scorecard of Ultimate Ocular Antioxidant by Swanson
Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
Ultimate Ocular Antioxidant contains three active ingredients: astaxanthin, lutein, and zeaxanthin. All three are antioxidants (substances that neutralize cell-damaging molecules) that concentrate in the eyes and skin.
The product also contains inactive ingredients—rice bran oil, gelatin, glycerin, sunflower oil, purified water, and natural tocopherols—which are typical softgel capsule materials.
Does it work?
Moderate evidence
Lutein and zeaxanthin are both possibly effective for age-related macular degeneration (AMD) and cataracts, two common eye conditions tied to aging. For astaxanthin, though, the evidence we hold is insufficient to rate it for any condition, including age-related macular degeneration, cognitive decline, aging skin, or Alzheimer disease.
That doesn't mean it doesn't work—it means the data aren't strong enough to establish effectiveness yet.
How safe is it?
Well-documented data
Lutein and zeaxanthin are generally well tolerated at typical supplement doses. Lutein up to 20 mg daily has not caused adverse effects in studies, and both are likely safe during pregnancy and lactation.
Astaxanthin is generally well tolerated in studies, though long-term safety data are limited. The most common side effects are abdominal pain, diarrhea, and red-colored stool.
Avoid astaxanthin during pregnancy and while breastfeeding—there isn't enough safety data to be sure.
Meds to double-check
Moderate interaction found
Check with your pharmacist if you take medications metabolized by CYP3A4 enzymes (Moderate severity)—astaxanthin may theoretically lower their levels. Also flag any drugs metabolized by CYP2B6 enzymes (Moderate severity) for the same reason.
Use the medication checker on this page to look up your exact prescriptions.
The bottom line
Scorecard at a glanceFully disclosed formula with some supporting evidence for its stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
This product is mainly for people interested in eye health, especially AMD or cataracts—lutein and zeaxanthin have some evidence behind them. If you take medications broken down by your liver (especially those metabolized by CYP3A4 or CYP2B6 enzymes), check with your pharmacist before adding this in, because astaxanthin may lower their levels.
Pregnant or nursing? Skip the astaxanthin content and talk to your doctor first.

Ultimate Ocular Antioxidant by Swanson Drug Interactions
HelloPharmacist Interaction Report
Ultimate Ocular Antioxidant by Swanson contains astaxanthin, which has moderate interactions with two groups of medications.
Astaxanthin may theoretically decrease levels of drugs metabolized by CYP3A4 enzymes—a broad group that includes many common medications. This effect comes from lab research showing astaxanthin can rev up CYP3A4 activity, though it hasn't been confirmed in actual human use.
Read the full breakdown — every affected drug type, severity by severity
Astaxanthin also theoretically may lower levels of drugs metabolized by CYP2B6 enzymes through the same mechanism. Again, this is based on test-tube findings and hasn't been documented in people.
Lutein and zeaxanthin, the product's other two active ingredients, have no known interactions documented in our data.
Altogether, these interactions span 661 individual medications. Before you start this product, run your exact prescriptions through the checker on this page—many everyday drugs depend on CYP3A4 or CYP2B6, and your pharmacist can tell you if any of yours are affected.
