Centered Mind has two active ingredients. Reishi mushroom extract is a fungal product used in traditional medicine for relaxation and immune support.
Theanine (branded here as AlphaWave) is an amino acid naturally found in tea leaves, included for its potential to support focus and calm without drowsiness. The capsule also contains inactive ingredients — hypromellose, silicon dioxide, stearic acid, and microcrystalline cellulose — which are standard binders and fillers.

The evidence for these ingredients is mixed. Theanine is rated Possibly Effective for cognitive function — it may help with focus and mental clarity.
For reishi mushroom, the data is thinner: it's rated Possibly Ineffective for cholesterol, and there's Insufficient Evidence for altitude sickness, Alzheimer's disease, and athletic performance. If you're looking for cognitive support, theanine has some backing; reishi's specific benefits here aren't well established in the data we hold.
How safe is it?

Both reishi and theanine are generally well tolerated when used short-term. Reishi may cause dizziness, dry mouth, itching, nausea, rash, stomach upset, sore throat, or runny nose — and rarely, insomnia or headache.
Some people can develop a respiratory allergy to reishi spores. Theanine may cause headaches, drowsiness, or increased dream activity.
Neither ingredient has enough safety data in pregnancy or while breastfeeding — the safety data advises against both in those situations.

AlphaWave
Antihypertensive Drugs
Theanine might lower blood pressure, potentiating the effects of antihypertensive drugs.
Animal research shows that theanine can lower blood pressure in spontaneously hypertensive animals. Theoretically, concomitant use of theanine and antihypertensive drugs might potentiate the antihypertensive activity.
Cns Depressants
Theoretically, theanine might have additive sedative effects when used in conjunction with CNS depressants. However, it is unclear if this concern is clinically relevant.
Theoretically, theanine may compete with glutamate and/or increase plasma gamma-aminobutyric acid (GABA) levels, which could cause CNS depression. In one clinical study, some subjects taking oral theanine reported drowsiness.
Serotonergic Drugs
Clinical studies regarding the effects of L-theanine on serotonin levels are conflicting. Some studies suggest it can increase serotonin levels in the brain while others report that it may decrease them. Nevertheless, there have been no reports of l-theanine being a causative agent in serotonergic-related side effects or serotonin syndrome.
Reishi Mushroom extract
Anticoagulant/Antiplatelet Drugs
Theoretically, high doses of reishi mushroom might increase the risk of bleeding.
A dose of 1.5 grams daily of reishi mushroom does not seem to decrease platelet aggregation, but a higher dose of 3 grams daily does.
Antidiabetes Drugs
Theoretically, reishi mushroom might have additive effects with antidiabetes drugs.
Animal research suggests that reishi mushroom decreases blood sugar. However, in patients with type 2 diabetes, taking reishi mushroom does not reduce fasting glucose levels, and its effects on glycated hemoglobin are inconsistent.
Antihypertensive Drugs
Theoretically, concurrent use of reishi mushroom with antihypertensive drugs might increase the risk of hypotension.
Reishi mushroom has shown hypotensive activity in animal research. Clinical evidence suggests that reishi mushroom reduces blood pressure in some, but not all, patients with hypertension.
