Saccharomyces boulardii is generally well tolerated in healthy adults but should be avoided by people who are critically ill or immunocompromised (those with weakened immune systems). Rarely, it has caused fungemia (yeast in the bloodstream), particularly in critically ill ICU patients with central lines, feeding tubes, or those on broad-spectrum antibiotics — most cases occurred when packets were opened or capsules broken.

Rarely, it may cause mild digestive complaints like cramps, gas, nausea, vomiting, or loss of appetite. Overall safety data is limited, so speak with your doctor before use.

Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
