Acetaminophen, Butalbital, Caffeine falls under STIMULANT DRUGS — the group 1,4-dmaa interacts with.

1,4-DMAA is thought to have stimulant and vasoconstrictive effects, which have been demonstrated in animal studies. Theoretically, taking 1,4-DMAA with stimulant drugs might increase the risk of adverse cardiovascular effects. Some stimulant drugs include amphetamine, caffeine, diethylpropion (Tenuate), methylphenidate, phentermine (Ionamin), pseudoephedrine (Sudafed, others), and many others.

Here's the simple version: the medicine you asked about (Tecnal, Esgic, and similar headache combos) already contains caffeine, which is a stimulant in its own right. 1,4-DMAA is also a stimulant that can tighten blood vessels and push up heart rate and blood pressure. Stacking two stimulants together can add up, so the worry is mostly on the heart and blood pressure side: a racing or pounding heart, jitteriness, restlessness, trouble sleeping, and higher blood pressure.

I'll be honest that this concern is theoretical, based on how these stimulants behave rather than solid human studies. That said, I'd steer clear of 1,4-DMAA entirely since the FDA considers it unsafe. Please check with your pharmacist or doctor before combining anything with this medication.

What Is Acetaminophen, Butalbital, and Caffeine Used For?

Brand names: Tecnal, Esgic Plus, Fiogesic, Triad, Repan, Esgic, Fioricet

This combination product — available as capsules, tablets, and oral solution under various names including Fioricet — is used to relieve the symptoms of tension headaches (also called muscle contraction headaches).

All supported products, including Fioricet capsules and the oral solution, share this same indication. Because butalbital can be habit-forming, extended or repeated use is not recommended, and these products are not established as safe or effective for treating headaches that come back frequently over time.
