Acetaminophen, Caffeine, Pyrilamine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group African Wild Potato interacts with.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the piece that matters in this combo: the caffeine in Midol is cleared by a liver enzyme called CYP1A2. In lab dishes, African wild potato seems to slow that enzyme down. If the same thing happens in people, caffeine could hang around longer than usual, which might leave you feeling jittery, anxious, restless, or with a racing heartbeat and trouble sleeping. The acetaminophen and pyrilamine parts aren't really the concern here.
I'll be honest with you though, this is only seen in test-tube studies, not in actual patients, so it's more of a theoretical caution than a proven problem. If you take this supplement regularly, it's worth a quick chat with your pharmacist before pairing it with caffeine-containing products.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Midol Max Strength, which combines acetaminophen, caffeine, and a gentle antihistamine called pyrilamine. In a test-tube study, African Wild Potato blocked a liver enzyme called CYP3A4 quite strongly. None of these three ingredients are prodrugs, so the theory is that blocking this enzyme could let them clear a little slower and build up slightly. In real life, that might mean a bit more caffeine jitteriness, trouble sleeping, or a touch more drowsiness from the pyrilamine.
That said, this is only lab data, and a human study with another CYP3A4 drug showed no real effect. So I wouldn't lose sleep over an occasional dose. Still, it's worth a quick chat with your pharmacist if you use the supplement regularly.
