Why this applies to Clomipramine

Clomipramine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group African Wild Potato interacts with.

In vitro research shows that African wild potato extract inhibits cytochrome P450 1A2 (CYP1A2). However, this effect has yet to be reported in humans. Until more is known, use with caution. Theoretically, concomitant use of African wild potato with CYP1A2 substrates might increase the risk for adverse effects from these substrates. Some of these drugs include amitriptyline (Elavil), haloperidol (Haldol), ondansetron (Zofran), propranolol (Inderal), theophylline (Theo-Dur, others), verapamil (Calan, Isoptin, others), and others.

Here's the gist: your body partly clears clomipramine using a liver enzyme called CYP1A2. In lab tests, African Wild Potato seemed to slow that enzyme down. If that also happened in real life, your clomipramine could be cleared a little slower and build up in your system, which can mean stronger effects and more side effects.

For clomipramine that might look like extra drowsiness, dry mouth, constipation, dizziness, a racing or irregular heartbeat, or trouble urinating. I want to be honest, though: this is based only on test-tube research, not on people, so it's a theoretical concern, not a proven one. Still, since clomipramine has a fairly narrow safe range, it's worth running this combo by your pharmacist or doctor before adding the supplement.

In vitro research shows that African wild potato extract inhibits cytochrome P450 2C9 (CYP2C9). However, this effect has yet to be reported in humans. Until more is known, use with caution. Theoretically, concomitant use of African wild potato with CYP2C9 substrates might increase the risk for adverse effects from these substrates. Some of these drugs include nonsteroidal anti-inflammatory drugs (NSAIDs) such as diclofenac (Cataflam, Voltaren), ibuprofen (Motrin), meloxicam (Mobic), and piroxicam (Feldene); celecoxib (Celebrex); amitriptyline (Elavil); warfarin (Coumadin); glipizide (Glucotrol); losartan (Cozaar); and others.

Here's the gist: in test-tube studies, African wild potato seems to block a liver enzyme called CYP2C9, which is one of the pathways that helps clear clomipramine from your body. If that enzyme is slowed down, clomipramine could build up a bit more than usual. Since clomipramine is an active drug (not a prodrug), more of it hanging around could mean stronger effects and more side effects, things like dry mouth, constipation, drowsiness, dizziness, a racing heart, or trouble urinating.

The honest part: this is only based on lab work, not real people yet, so it's theoretical. I wouldn't panic, but I would loop in your pharmacist or doctor before combining them, especially since clomipramine already has a lot of side effects to watch.

Clomipramine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro evidence shows that African wild potato extract can inhibit cytochrome P450 3A4 (CYP3A4) enzyme activity by up to 86%. Theoretically, African wild potato might increase levels of drugs metabolized by CYP3A4. However, in human research, African wild potato does not seem to affect the pharmacokinetics of Efavirenz, a CYP3A4 substrate. Until more is known, use African wild potato cautiously or avoid in patients taking drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist: your body uses a liver enzyme called CYP3A4 to help clear clomipramine. In test-tube studies, African wild potato slowed that same enzyme quite a bit. If that happened in real life, clomipramine could pile up in your system and hit harder than usual. With a tricyclic like this, that can mean more dry mouth, constipation, blurry vision, dizziness, a racing or irregular heartbeat, drowsiness, or trouble urinating.

The reassuring part: this is based on lab data, not people, and in one human study the supplement didn't change levels of a similar CYP3A4 drug. So the real-world risk may be small. Still, since clomipramine isn't a forgiving drug, please check with your pharmacist or doctor before pairing the two.

To qualify for treatment, the obsessions or compulsions must cause significant distress, take up a lot of time, or noticeably interfere with work, school, or social life.
