Xultophy is a once-daily injection that combines a long-acting insulin (degludec) with liraglutide, and both of those already work to bring your blood sugar down. Taking aloe by mouth may nudge your blood sugar a little lower too, so the concern here is that the two could add up and push you into hypoglycemia (low blood sugar). That can show up as shakiness, sweating, dizziness, a fast heartbeat, confusion, or feeling suddenly weak.

The good news: this is based mostly on early lab and small human studies, so it is more of a 'keep an eye out' than an alarm. If you use oral aloe regularly, it is worth checking your sugars a bit more closely at first and mentioning it to your pharmacist or doctor. Aloe gel used on the skin is not a concern here.
