Artichoke and Treprostinil (Inh)

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTIHYPERTENSIVE DRUGS

Why this applies to Treprostinil (Inh)

Treprostinil (Inh) falls under ANTIHYPERTENSIVE DRUGS — the group Artichoke interacts with.

Theoretically, artichoke leaf extract may increase the risk of hypotension when taken with antihypertensive drugs.
A meta-analysis of small clinical studies in patients with hypertension shows that taking artichoke can reduce systolic blood pressure by around 3 mmHg and diastolic blood pressure by around 2 mmHg when compared with placebo.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: inhaled treprostinil (Tyvaso) is a vasodilator, meaning it relaxes and widens blood vessels. Artichoke leaf extract, on its own, may nudge your blood pressure down a little too (studies suggest roughly 3 points on the top number and 2 on the bottom). Put the two together and, in theory, those small effects could stack up.

What that might feel like: more lightheadedness, dizziness, flushing, or a headache, especially when you stand up quickly. To be clear, this is a theoretical concern based on how both work, not something proven to cause trouble. The blood pressure dip from artichoke is mild. Still, since you're managing pulmonary hypertension, it's worth a quick chat with your pharmacist or doctor before adding it.

From our pharmacist’s overview on the Treprostinil (Inh) page

“Inhaled treprostinil (Tyvaso) relaxes blood vessels in the lungs to treat pulmonary hypertension, and it also blunts platelet clotting, which is why this list of 275 ingredients warrants real vigilance rather than a shrug. Two themes cover almost everything.”
