Diltiazem, Enalapril falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Astaxanthin interacts with.

Theoretically, astaxanthin may decrease levels of drugs metabolized by CYP3A4.
In vitro research shows that astaxanthin induces CYP3A4 enzyme activity in human hepatocytes. This effect has not been reported in humans.

Your combo pill contains diltiazem (a blood pressure and heart rate medicine) plus enalapril. The diltiazem part is processed by a liver enzyme called CYP3A4, and that is where astaxanthin could theoretically play a role. In lab dishes, astaxanthin seemed to speed up this enzyme, which could clear diltiazem out faster and make it work a little less well. In real life that might mean your blood pressure or heart rate isn't as well controlled as expected.

Here's the honest part: this comes only from test-tube and cell studies, and it has never actually been shown in people. So I wouldn't lose sleep over it. Still, since both of these affect your heart and pressure, just give your pharmacist or doctor a heads-up before adding astaxanthin so they can keep an eye on your numbers.
