Alcohol can impair gluconeogenesis and may increase the risk of acute hypoglycemia when used concomitantly with antidiabetes drugs. However, the carbohydrates in beer may also worsen glycemic control in patients with diabetes.

Mixing beer with Kombiglyze (saxagliptin plus metformin XR) can push your blood sugar in unpredictable directions. Here's the tricky part: alcohol blocks your liver from making new sugar (gluconeogenesis), so when paired with a diabetes medicine that already lowers glucose, you can drop too low. Signs of low blood sugar include shakiness, sweating, confusion, a racing heart, or feeling faint, and these can be easy to confuse with simply feeling buzzed.

On the flip side, beer carries carbs that can nudge your sugar up too. There's also a small added concern: metformin and heavy alcohol together raise the risk of a rare but serious buildup of lactic acid. None of this means you must avoid one drink, but please check with your pharmacist or doctor about what's safe for you.

Your Kombiglyze contains two medicines, and the one that matters here is the metformin part. Metformin can slightly raise lactic acid in your blood, and heavy drinking pushes that risk higher. The concern isn't a single beer with dinner for most people. It's drinking a large amount at once, or regular heavy drinking, that can lead to a rare but serious problem called lactic acidosis.

Watch for warning signs like deep tiredness, muscle aches, trouble breathing, stomach pain, or feeling very cold or dizzy. Alcohol can also drop your blood sugar, so know your low-sugar symptoms too. Please chat with your pharmacist or doctor about how much, if any, beer is reasonable for you given your kidneys and overall health.
