Caffeine (OTC drug) falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Beet interacts with.
In vitro research suggests that betanin, the major pigment in beet, competitively inhibits CYP3A4 in a dose-dependent manner similarly to strong CYP3A4 inhibitor ketoconazole.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest picture: this concern comes from lab dish research, not from real people. In those test tube studies, betanin (the red pigment in beets) slowed down an enzyme called CYP3A4. The idea is that if that enzyme is blocked, drugs it clears could build up a little. With caffeine, though, I want to point out that caffeine is mostly broken down by a different enzyme, CYP1A2, not CYP3A4. So even in theory, beets are unlikely to have much of a meaningful effect on your caffeine levels.
If anything did stack up, you'd notice the usual caffeine over-stimulation: jitteriness, a racing heart, trouble sleeping, or feeling wired. But honestly, normal food amounts of beets shouldn't cause this. If you're using concentrated beet supplements and feel off, just check with your pharmacist.

Here's the gist for combining beet with caffeine pills. Caffeine is processed by a liver enzyme called CYP1A2. Some early lab research hints that beet might speed up that enzyme a bit. If that happened in real people, your body could clear caffeine slightly faster, meaning the alertness boost from a pill like Vivarin might wear off a touch sooner or feel a little weaker.
Honestly, I wouldn't lose sleep over this one. The evidence is only from test-tube and animal studies, the effect is rated minor, and beets as food are very unlikely to matter. If you rely on caffeine and notice it acting differently after adding beetroot supplements, just mention it to your pharmacist or doctor.
