Beet and Prednisolone

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Prednisolone

Prednisolone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Beet interacts with.

Theoretically, beet might increase the levels of CYP3A4 substrates.
In vitro research suggests that betanin, the major pigment in beet, competitively inhibits CYP3A4 in a dose-dependent manner similarly to strong CYP3A4 inhibitor ketoconazole.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Your body uses an enzyme called CYP3A4 to help break down prednisolone and clear it out. There's some early lab work suggesting that betanin, the pigment that gives beets their deep red color, can slow that enzyme down. If that happened in real life, prednisolone could hang around a bit longer and its effects could feel a little stronger.

For prednisolone, that might mean more of the usual steroid side effects, things like trouble sleeping, mood swings, a bump in blood sugar, fluid retention, or stomach upset. Here's the honest part though: this is only seen in test-tube studies, not in people, and eating beets or drinking a normal glass of beet juice is unlikely to cause a problem. Concentrated beet supplements are more of a question mark. If you take prednisolone regularly and want to use beet supplements, just run it by your pharmacist or doctor first.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Prednisolone page

“Prednisolone quiets inflammation and dampens the immune system, and its list of 202 interacting supplement and herbal ingredients, four of them rated major, deserves a careful read rather than a shrug. Grapefruit can push prednisolone levels up by blocking its breakdown, St.”
