Fosaprepitant falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Beet interacts with.

Theoretically, beet might increase the levels of CYP3A4 substrates.
In vitro research suggests that betanin, the major pigment in beet, competitively inhibits CYP3A4 in a dose-dependent manner similarly to strong CYP3A4 inhibitor ketoconazole.

Here's the gist: fosaprepitant (Emend) is given as an IV before chemo to prevent nausea and vomiting. In your body it quickly turns into aprepitant, which is broken down largely by an enzyme called CYP3A4. Lab studies hint that a pigment in beets (betanin) can block that same enzyme, similar to how some strong inhibitors work. In theory, that could let aprepitant linger a bit longer and feel a little stronger, which might mean more of its usual effects like tiredness, hiccups, or constipation.

But I want to keep this in perspective. This concern comes only from test-tube research, not from people, and it's rated as a theoretical interaction. Eating beets or drinking some beetroot juice is very unlikely to cause a problem. Concentrated beet supplements are less studied. If you take those, just mention it to your oncology team or pharmacist.
