Fosaprepitant falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Horny Goat Weed interacts with.

Theoretically, horny goat weed might increase the effects and side effects of CYP3A4 substrates.
In vitro, horny goat weed extract inhibits CYP3A4 and suppresses CYP3A4 mRNA expression. This effect has not been reported in humans.

Here's the short version on combining horny goat weed with Fosaprepitant (Emend Injection), the IV anti-nausea medicine you usually get with chemotherapy. Fosaprepitant converts to aprepitant in your body, and that active drug is handled by a liver enzyme called CYP3A4. In test-tube studies, horny goat weed appears to slow down that same enzyme. If that held true in people, aprepitant could hang around longer than expected, which might mean more of its typical effects like fatigue, hiccups, mild belly upset, or headache.

The big caveat: this is only laboratory data and has never been shown in actual humans, so I wouldn't lose sleep over it. Fosaprepitant is also given short-term in a clinic setting. Still, please mention any supplements to your oncology team or pharmacist before your infusion so they can keep an eye on things.
